Common sources of ethical standards are compared and contrasted as to their relative strengths and weaknesses.  The value of codes of conduct promulgated by professional bodies is emphasized, and several examples are provided. Some common views of ethical standards are presented and discussed in order to underline flawed thinking regarding ethical practice leading to mistakes and violations.  An extensive case study is presented regarding the Kansas City Hyatt Regency Walkway collapse.  This case study highlights the relative impact of technical flaws versus professional negligence in generating this catastrophe.
